CIFC Health's Primary Care Internal Medicine curriculum is framed around the Patient Centered Medical Home (PCMH) model, which puts the "patient at the center of the health care system, and includes family, medical, dental, vision and behavioral health providers, specialists, ancillary staff, and the wider community" (National Committee for Quality Assurance).
CIFC Health is recognized as a Level 3 Patient Centered Medical Home (the highest rating) by the National Committee for Quality Assurance (NCQA). In the PCMH model, Residents participate in clinician-led teams to coordinate patients' care, particularly for prevention and chronic conditions. The patient-centered medical home model facilitates greater care coordination and communication, resulting in a higher quality of care for the patient.
Residents' ambulatory experiences take place at CIFC Health where residents develop long-term relationships with a panel of their own patients. In addition, during structured ambulatory rotations, which are called Medical Home Blocks, Residents acquire additional knowledge and skills in primary care and specialty outpatient medicine. It is through the composite of these experiences that Residents attain and develop the knowledge and skills that are necessary for delivering outstanding patient care.
