Making Your Appointment

Please call (203) 743-0100 to make an appointment. Same day appointments are available if needed. If you need to cancel an appointment, please call us 24 hours before your appointment.

How to prepare for your appointment:

  • Be sure to bring your photo ID, proof of address and insurance information.
  • If you don't have insurance, or have limited income, you can complete an application for our Discounted Fee Schedule Program. Please bring proof of income to finish this application.
    • NOTE: You may also be eligible for HUSKY or insurance subsidies through Access Health CT. An eligibility specialist can help you determine if you're eligible. Click here for more information.

Our Service Area & Locations:

We serve the towns of Danbury, Bethel, Brookfield, Bridgewater, New Fairfield, New Milford, Newtown, Redding, Ridgefield and Sherman.

In 2019, 1.83% of all GDCHC patients were screened for Social Determinants of health factors. As of 06/30/2020, 25.78% of all GDCHC patients were screened for Social Determinants of health factors.

GDCHC has increased Social Determinants of Health Screening by 24 percentage points from 2019 to YTD 2020


This year, nearly 22 percent more children (ages 6-9) than in 2018, who were at moderate to high risk of having tooth decay or cavities, received dental sealants through the Greater Danbury Community Health Center Dental Service, as of Aug. 31.


The Health Center's primary sites are located at 120 Main and 70 Main Street in downtown Danbury, and 57 North Street, also in Danbury; providing comprehensive pediatric and adult primary medical (including OB/Gyn), dental and behavioral health care services.  In 2017 we opened a dedicated adult behavioral health site at 152 West St., Danbury, CT 06810.  We also maintain five School-Based Health Center sites:

  2. Danbury High School
  3. Rogers Park Middle School, Danbury
  4. Broadview Middle School, Danbury
  5. Newtown Middle School

The Health Center also sponsors the Region's Women, Infants and Children (WIC) Nutrition Program enrollment services, which is located at 80 Main Street in downtown Danbury.

What is an FQHC?

A federally qualified health center (FQHC) is a community-based and patient-directed organization that serves populations with limited access to health care. Every FQHC:

  • Is located in or serves a high need community.
  • Is governed by a community board composed of a majority (51% or more) of health center patients who represent the population served.
  • Provides comprehensive primary health care services as well as supportive services (education, translation and transportation, etc.) that promote access to health care.
  • Provides services available to all with fees adjusted based on ability to pay.
  • Meets other performance and accountability requirements regarding administrative, clinical, and financial operations.
